Discussion:
NCX 470 is a novel nitric oxide-donating prostaglandin analog that has a dual mechanism of action with increase of uveoscleral outflow rate, likely from the effect of bimatoprost,1 and improvement in outflow facility, likely from the effect of nitric oxide on the trabecular meshwork.2, 3 Approximately 58% of the IOP reduction is due to uveoscleral outflow increase and 42% is from outflow facility improvement. Despite potential episcleral vasodilation from nitric oxide, there was no significant effect on EVP.

Conclusion:
In healthy adults without glaucoma, NCX 470 0.1% dosed once daily significantly lowered IOP by increasing outflow facility and uveoscleral outflow.


Discussion:
In the Denali clinical trial, NCX 470 demonstrated fast, powerful, and consistent IOP reduction over 3 months that was noninferior to latanoprost and statistically superior at 3 of 6 time points. NCX 470 was safe and well tolerated with low discontinuation rate.



Conclusion:
NCX 470 is new NO-donating bimatoprost that is in development for IOP lowering in patients with OAG and OH. In the phase III Denali trial, NCX 470 was non-inferior to latanoprost and was safe and well tolerated.
